Providence Medical Group (PMG) Annual Wellness nurse practitioners are integral members of Providence's care team, serving as a knowledgeable expert in preventive care, risk assessment, and population health. In this role you will see patients for their Annual Wellness Visits (AWV) which includes review of medical and family history, physical exams & screenings to detect depression, cognitive impairment, functional ability & fall risks. As a collaborator to the patient's primary care team, you will assist in detailed chart reviews, proper coding and data analysis to help identify all medical conditions that require clinical assessment and to reduce missed gaps in care. This position partners with PMG clinics throughout the Portland metro area.
