Hospitalist for Hospital-at-Home Program– San Antonio, Texas

University Health / University Medicine Associates is presently seeking a board-certified family or internal medicine physician with a passion for hospital medicine to provide care through our “Hospital-at-Home” (HAH) program.  This unique program allows eligible patients who need acute/inpatient care to receive treatment at home instead of the hospital.  This program has proven to reduce costs, improve outcomes and increase patient satisfaction. 

Position details:

• Work onsite at University Hospital in dedicated office
• Day-shift – 7 days on / 7 days off
• Full-time advanced practice provider support
• Conduct the History & Physical Exam on patients transitioning to the HAH program when admitted from the emergency room or meet with patients currently inpatient before transfer to HAH
• Conduct one telemedicine visit with the HAH patients per day (Any needed home visits are conducted by a registered nurse or community medical team)
• Complete discharge summaries on patients discharged after completing stay from HAH
• Rounding in hospital to assess patients who may qualify for the program
• When working must be available for after hour phone calls (minimal) if the patient needs to be admitted back to the hospital, the patient will transfer back to the inpatient hospitalist service
• Expectation is 18 tele-visits and rounds on the floor for transfers to HAH
