Providence St. Joseph's Program of All-inclusive Care for the Elderly (PACE) is seeking a part-time or a full-time nurse practitioner with experience in Adult Primary Care, Gerontology or Family Medicine to join a compassionate and dedicated interdisciplinary team providing primary care services to patients 55 and older throughout Napa, California. Here, you will primarily focus on providing full-spectrum outpatient geriatric and palliative care services at the PACE clinic, contracted nursing homes, as well as patient homes. You will work alongside a highly supportive team consisting of geriatricians, RNs, Social Workers, PTs, OTs, dieticians and chaplains, to provide health care services to a vulnerable patient population in Napa and surrounding communities in northern California.
