Candidate Requirements

• Board Certified REQUIRED
  
• Active Idaho License or IMLC LOQ REQUIRED
  
• Proof of COVID vaccination at time of presentation REQUIRED
  
• Must be available for July 10, 11, 12 REQUIRED
  
• NPDB (dated within last 30 days) REQUIRED
