ECFMG required for foreign medical graduates.Clinical Experience: N/ALicense: Active unrestricted VA Medical License and DEA w/ VA address & schedules 2,2N,3,3N,4 & 5Board Certification: American Board of Internal Medicine (ABIM), American Board of Family Medicine (ABFM), and American Board of Medical Specialties(ABMS).Merit Badge: ACLSImmunizations: MMR/Varicella (2) Vaccines or titers with values indicating immunity for MMR & Varicella. TB (past 12 months)or a QuantiFERON Gold indicating immunity within three months. 2nd TB test to be completed prior to activation and privileges given. Chest x-ray required within 3 months if positive for TB or positive QuantiFERON. FLU within current season.Case Log: Case Logs for the last two years, in addition any documentation for special training for Special Procedures.CME/CEU: N/ASpecific Facility Procedures: Vent management required.Malpractice Coverage: Per Loss Event $2,7M/$8,1M Annual AggregateAdditional Details: Required to be a supervising physician for NP's and PA's.
