Job Details

OnCall Solutions is currently seeking Board Certified / Board Eligible Hospitalists and Internal Medicine physicians for a cross-credentialed locum tenens program across three inpatient facilities in Central California.

A key feature of this opportunity is that providers will be cross-credentialed across all three sites, allowing flexibility to rotate between high-acuity medical centers and a lower-volume surgical hospital while maintaining consistent employment within one coordinated system.

Below is a detailed overview of each opportunity:

Position 1 High-Acuity Acute Care Medical Center



This is a fast-paced, high-volume hospitalist service managing medically complex patients with significant operational demands.

Volume / Acuity

  • Average daily census: + patients
  • Admissions: 40 per 24 hours
  • Swing admits: 7 9
  • Night cross-cover census: patients
  • Night admissions: 6 7 per nocturnist
ICU / Clinical Structure

  • Closed ICU (ICU limited to intubated and select pressor-level patients)
  • Hospitalists manage step-down and high-acuity non-ICU patients
  • Rapid response responsibilities primarily handled by admitting team
  • Nocturnists also respond to rapid response events
Staffing Model & Shifts

  • Day Rounders: 7a 7p ( physicians depending on seasonality)
  • Day Admitter: 7a 7p (1)
  • Swing MD 1: 1p 1a (1)
  • Swing MD 2: 3p 3a (1)
  • Night MDs: 7p 7a (2)
  • APP Day Support: limited / evolving model
  • APP Night Cross-Cover: 7p 7a (cross-cover only; no admissions)

  • Hospitalists supervise APPs and manage transitions of care, admissions, and consult coordination

Position 2 Level I Trauma & Tertiary Referral Center



A large academic-style hospital with high acuity, trauma services, and broad specialty coverage.

Volume / Acuity

  • Average daily census: 330+
  • Admissions: 53 per 24 hours
  • Swing admits: 7 9
  • Night admissions: 7 9
  • Night cross-cover census: 330+
ICU / Clinical Structure

  • Multiple ICUs (Adult, Neuro, Surgical)
  • Closed ICU model with critical care oversight
  • Hospitalists respond to rapid responses
  • Code airway management and procedures handled by critical care teams
Staffing Model & Shifts

  • Day Rounders: 7a 7p ( physicians depending on seasonality)
  • Day Admitter: 7a 7p (1)
  • Swing MD 1: 11a 11p (1)
  • Swing MD 2: 2p 2a (1)
  • Night MDs: 5p 5a (1), 7p 7a (1), 8p 8a (1)
  • APP Day Rounders: 7a 7p (2)
  • APP Swing: 12p 12a (1), 1p 1a (1)
  • APP Night Cross-Cover: 7p 7a (1; no admissions)

Position 3 Surgical Specialty Hospital (Low Volume / Elective Care)



A structured inpatient surgical hospital focused on scheduled procedures with no emergency department.

Volume / Acuity

  • Average census: patients
  • Night admissions: 7
  • Primarily elective/scheduled surgical cases
  • Lower weekend volume
ICU / Clinical Structure

  • Small ICU (9 beds), open structure
  • Surgeon-led care with hospitalist consultation
  • Anesthesia manages airway and procedural support
  • Hospitalists respond to codes and rapid response events
Staffing Model & Shifts

  • Day MD: 7a 7p (2 physicians)
  • Night MD: 7p 7a (1 physician; cross-cover responsibilities included)
  • No dedicated APP coverage
Key Practice Environment Notes

  • Epic EMR
  • Predictable surgical inpatient workflow
  • No ED or unscheduled admissions
  • Hospitalist role includes cross-cover of medical issues for surgical patients
  • Strong coordination with surgical and anesthesia teams
General Program Details (All Positions)

  • Internal Medicine and Family Medicine Hospitalists eligible
  • Board Certified or Board Eligible (within standard eligibility timeframe)
  • ACLS required
  • Must have recent inpatient and ventilator management experience (within 24 months)
  • Credentialing timeline: days (can extend up to 120 days during peak periods)
  • Temporary privileges available once file is fully complete and cleared
  • Physicians will be cross-credentialed across all three facilities for rotational flexibility
