Core Requirements

• Board Certified in OB/GYN (required)
• Active California medical license (or in process) (required)
• DEA registration must be obtained prior to start (required; no exemptions)
• Clean malpractice and license history (highly preferred)
• Most recent NPDB Self Query Report (within 30 days of presentation) (required; no exemptions)

• Indicate interest in full-time or part-time employment
• Indicate visa status/need for sponsorship (J-1 or H-1B)
• Confirm DEA will be obtained prior to start

Other

• Good standing with no disciplinary actions
