We're looking for a full-time Attending Physician to provide compassionate, patient-centered care to residents in skilled nursing facilities across the Lima, Ohio area. This role is perfect for a physician who values clinical autonomy, enjoys working within a collaborative care team, and wants to make a real difference in the health and quality of life of older adults. You'll oversee medical care for assigned residents, work closely with Nurse Practitioners and Physician Assistants, and help lead quality initiatives - all without on-call or overnight duties.

What you'll do:
- Serve as the Attending Physician for residents in assigned skilled nursing facilities, diagnosing and managing acute and chronic conditions.
- Partner with NPs and PAs to deliver team-based, coordinated care and provide clinical oversight.
- Develop individualized care plans and participate in transitions of care.
- Engage in Chronic Care Management, Transitional Care Management, Remote Patient Monitoring, and other quality improvement efforts.
- Use streamlined documentation tools, including an intuitive EHR, voice dictation, AI-powered scribe technology, and UpToDate for real-time clinical support.

Who we're looking for:
- MD or DO with an active, unrestricted U.S. medical license or eligibility for licensure.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine.
