Working collaboratively with physician partners and an interdisciplinary care team, Advanced Practice Providers (APPs) oversee and manage the clinical care of patients receiving care at home.

We are seeking an experienced Physician Assistant (PA) or Acute Care Nurse Practitioner (ACNP) to provide virtual clinical oversight, complex patient management, and care coordination while delivering exceptional patient-centered outcomes.

• 7:00 AM - 5:00 PM PST

Position Overview

Role Summary
As a Virtual Hospitalist APP, you will play a key role in delivering high-quality care to patients in the home setting. In collaboration with physician partners and interdisciplinary care teams, you will conduct comprehensive telemedicine assessments, evaluate patients for admission to the Advanced Care at Home (ACAH) program, and support safe, effective care delivery throughout the patient journey.

Clinical Partnership and Program Advocacy
You will also serve as a clinical ambassador for ACAH by partnering with hospital-based and virtual care teams to identify appropriate patients, promote timely admissions, and strengthen awareness of this innovative care model.

Key Responsibilities

• Patient Evaluation and Admission: Conduct admission evaluations, review clinical information, and determine whether patients are appropriate for the ACAH program.
• Care Planning: Develop individualized plans of care in collaboration with physician partners and interdisciplinary care teams.
• Admission Coordination: Identify appropriate patients in acute care settings and facilitate timely transition into the ACAH program.
• Stakeholder Engagement: Build strong relationships with hospital-based physicians, nurses, case managers, and virtual care teams to promote awareness and appropriate utilization of ACAH.
• Quality and Patient Experience: Support safe, high-quality, patient-centered care throughout the patient journey.

Key Responsibilities
Virtual Patient Assessment & Admission

• Conduct comprehensive virtual patient evaluations in collaboration with physician partners.
• Review medical histories, diagnostic results, and clinical documentation to support admission decisions.
• Evaluate patient eligibility and clinical fit for the Advanced Care at Home program.

Remote Patient Management

• Develop, implement, and adjust individualized plans of care based on each patient's evolving clinical needs.
• Coordinate care across interdisciplinary teams to support safe, timely, and patient-centered outcomes.

Collaboration & Communication

• Coordinate effectively with physicians, specialists, virtual care nurses, home health teams, and other care partners to support seamless patient care.
• Maintain accurate, timely, and complete documentation in the electronic medical record (Epic) to support continuity of care and program standards.

Qualifications
Required

• Master's Degree from an accredited Physician Assistant or Nurse Practitioner program.
• NCCPA Certification (PA) or Acute Care NP Certification.
• Active California PA or NP license.
• Active DEA registration in California.
• Must reside in California.
• At least 2 years of NP / PA clinical experience and at least 1 year of specialty experience within the last 2 years:
  • Hospitalist Medicine
  • Emergency Medicine
  • Acute Care

Preferred

• Telemedicine or virtual care experience.
• Hospital-at-Home
• EPIC proficiency.
